To register Golfers, please first indicate TOTAL number of players next to registration cost ($175 per player, up to four golfers).

Each golfer registration includes green fees, a golf cart, a box lunch, and dinner reception.

For each golfer (up to 4 players), list full name and their handicap. As you fill in each name, additional golfer fields will appear. If they do not have an official handicap, please indicate their average score for the last five rounds.

If you are a local business or community member interested in supporting Temple Emanu-El or Yale New Haven Smilow Cancer Hospital, we invite you to donate a silent auction item or raffle item.
